What Is Endovenous Laser Treatment (EVLT Laser)?


In daily life, varicose veins are a common venous system disease. The common symptom is that the superficial blood vessels are looking like sputum, and the varicose skin is obviously lumpy or nodular. The main reason for the formation of varicose veins is that the congenital vascular wall membrane is relatively weak, or the same posture is rarely changed for a long time, resulting in blood accumulation in the lower limbs.

Varicose veins not only affect the appearance, if left untreated, but they may also develop into swelling, fatigue, skin changes, discoloration, inflammation, and even venous ulcers.
 

Optimal Wavelength for Phlebology
1940nm has the highest absorption coefficient in water as compared to the Generation 1 to Generation 3 laser diodes (808nm, 980nm, and 1470nm).

This means that there is a higher absorption of the laser energy by the interstitial water in the venous walls. Studies have proven that the efficiency of 1940nm laser to be the most superior to EVLA than the others. In fact, it has been proven that the 1940nm laser diode has an absorption rate several times higher than 1470nm laser and dozens of times higher than 980nm laser.

 

ADVANTAGES OF USING 1940nm RADIATION IN EVLT
As can be seen in the diagram, for 980nm radiation, the absorption coeffcient is higher for hemoglobin than for water. For 1470nm, especially 1940nm, the opposite is true and water absorbs light radiation many times better than hemoglobin. Both of these chromatophores are components of human tissues, including blood and vascular walls. Lasers with a wavelength of 980nm have been used on a large scale for over 20 years and the average power used during treatments is 10-12W. Using 1470nm wavelength, smaller powers are needed – at 6-8W, and for 1940nm wavelengths 4-6W is enough to perform an effective surgery. Reducing the amount of power needed, makes these lasers much safer and predictable tools for the doctor, while giving patients faster and more comfortable recovery.

 

How is Endovenous Laser Treatment done?
 It is an office procedure.
An ultrasound may be performed to mark areas planned for treatment before the procedure.
 Patient rests comfortably on the table.
Treated leg is cleansed with a disinfectant soap and draped with sterile sheets.
Small area is numbed and a tiny incision is made in the skin.
A catheter bearing a laser fiber is inserted in the diseased vein under ultrasound guidance.
 Dilute local anesthetic is injected around and along the treated vein.
The laser is activated and withdrawn very slowly causing thermal damage and closure of the diseases vein.
 Edges of previously made small incision are subsequently approximated with surgical strips (no stiches).
 Compression stocking are applied and patient rests a few minutes.
 Patient returns to normal activities immediately after procedure.


The Three-Step Mechanism of Action

1. Precision Energy Delivery

The 1940 nm laser beam penetrates the skin and is preferentially absorbed by intracellular and extracellular water within the target vein wall. This selective absorption ensures high photothermal efficiency with minimal dispersion into surrounding structures.

2. Direct Vessel Wall Closure

Once absorbed, the laser energy is rapidly converted into heat. Unlike wavelength modalities targeting hemoglobin, thermal energy is concentrated directly on the venous wall rather than diffusing through the blood column. This results in instantaneous dehydration, collagen contraction, and coagulative necrosis of endothelial and supportive layers-effectively creating a "precision photothermal weld" that occludes the vessel.

3. Progressive Fibrosis and Clearance

The occluded vein subsequently undergoes fibrosis, gradually transforming into a fibrous cord that is naturally resorbed by physiological metabolic processes over the following weeks to months, leading to permanent resolution.
