The Orthodontic Palatal Expander is a fixed or removable intraoral appliance designed for slow or rapid expansion of the maxillary arch. It is commonly used in growing patients to correct transverse discrepancies, unilateral or bilateral posterior crossbites, and to create sufficient arch perimeter for alleviating crowding.
- Rapid Palatal Expander (RPE): Cemented to the molars with a mid-palatal screw, activated daily to separate the mid-palatal suture.
- Slow Palatal Expander: Uses springs or screw with lower force, achieving skeletal expansion more physiologically.
- Hybrid Expander: Combines skeletal anchorage (e.g., mini-implants) with conventional bands for minimal dental side effects.
- Posterior crossbite due to maxillary constriction
- Mild to moderate anterior crowding
- Cleft palate patients needing arch expansion before bone grafting
- Pre-adjustment for space closure or other orthodontic mechanics
- Impression and bite registration taken
- Appliance fabricated on working model
- Patient adjusts screw as prescribed (e.g., 0.25 mm per day)
- Regular follow-up to monitor expansion progress and suture opening
- After desired expansion, appliance is kept passive for retention (3-6 months) or replaced with a removable retainer
- Soft diet is encouraged during active expansion
- Maintain meticulous oral hygiene around bands and acrylic
- Monitor for screw jamming or tissue impingement
- Not indicated in patients with completed transverse growth unless combined with surgical assist
Each expander is individually sealed and sterilized. Orthodontic labs can specify screw type (e.g., Leone, Dentaurum), wire gauge, and acrylic color (clear, pink, or tooth-shaded).
