As long as the limb leads are away from the shoulders and hips, their position has little effect on the electrocardiogram. Connecting the limb leads closer to the heart (such as near the elbow and knee joints) can reduce the electrical impedance of the limbs and make the electrocardiogram clearer.

However, studies have found that the distance of limb leads can alter the limb conduction voltage and Q wave duration. For example, in the modified Cornell criteria for left ventricular hypertrophy (LVH), a RavL>1.2 mV is diagnosed as LVH and is influenced by the distance of the limb leads. There is still controversy over the position of limb leads, so it is recommended to place the leads on the wrist or ankle.

In fact, for chest leads, the most important thing is the repeatability of the electrocardiogram. In men, only half of the electrocardiograms can be repeated (two lead positions within 1cm), and in women, it is even less.
